Testosterone esters in oil are absorbed slowly from the injection site, producing a peak within days and a gradual decline over the following week — the pattern that defines the dosing interval.

HOW CYPIONATE WORKS
Testosterone cypionate is testosterone with a cypionate ester attached, suspended in oil. Injected into muscle or subcutaneous tissue, it releases gradually as enzymes cleave the ester. What enters circulation is testosterone; the ester simply controls the timing.
The cypionate ester slows absorption from the injection site, which is what makes weekly rather than daily dosing practical.
Smaller weekly doses produce less variation between peak and trough than larger doses given every two weeks.
External testosterone reduces the pituitary's LH and FSH signal, which can reduce sperm production and testicular size.
Testosterone, estradiol, hematocrit, and PSA are followed throughout treatment, with hematocrit the most common reason for adjustment.
