In men whose gonadotropins were suppressed by testosterone, low-dose hCG maintained intratesticular testosterone in a dose-dependent manner — the testes stayed active despite the external supply.
Human Chorionic Gonadotropin Injection

HCG mimics luteinizing hormone, the signal that tells the testes to produce testosterone. It is most often used alongside or after testosterone therapy to keep that pathway active.
HCG is not approved for weight loss. Requires monitoring; estradiol can rise on hCG. Prescription only; dispensed after medical evaluation.

HOW HCG WORKS
Glucose control.
HCG is structurally similar to luteinizing hormone and binds the same receptor on the Leydig cells of the testes. That makes it a way to stimulate testicular testosterone production directly, independent of whether the pituitary is still sending its own signal.
HCG binds the LH receptor in the testes, prompting the same downstream testosterone production the pituitary signal would trigger.
Because external testosterone suppresses the pituitary signal, hCG is often added to keep intratesticular testosterone and testicular volume from declining.
Men who want to preserve sperm production while on testosterone therapy, or who are planning to come off it, are the typical candidates.
Estradiol can rise on hCG. Bloodwork is part of the protocol, not optional.
