It requires arousal to work

Sildenafil inhibits PDE5, the enzyme that breaks down cGMP. Since arousal is what triggers cGMP production in the first place, the medication amplifies an existing signal rather than generating one.

Arousal triggers nitric oxide release in penile tissue, which produces cGMP, which relaxes smooth muscle and allows blood flow in. An enzyme called PDE5 breaks cGMP down. Sildenafil inhibits PDE5, so the signal persists long enough to produce and maintain an erection.
