HOW SPIRONOLACTONE WORKS
Glucose control.
Spironolactone was developed as a potassium-sparing diuretic, but it also binds androgen receptors without activating them. In skin, that blocks the androgen signal that drives sebaceous glands to produce oil — which is why it is used for hormonally driven acne, unwanted hair growth, and androgenetic hair loss in women.
Occupying the receptor without activating it reduces the androgen signal reaching sebaceous glands and hair follicles.
Less oil means fewer clogged follicles, which addresses hormonal acne closer to its source than topical treatment does.
Meaningful change is generally assessed at three to six months. The SAFA trial found the gap over placebo continued widening through 24 weeks.
