In a randomized crossover trial in fibromyalgia, participants reported greater reduction in pain during the low-dose naltrexone period than during placebo, in a study the authors themselves describe as small and preliminary.
Low-Dose Naltrexone HCL Tablets
$79/mo*
At about a tenth of the standard dose, naltrexone behaves differently — briefly occupying opioid receptors and dampening immune signaling in the central nervous system.
Low-dose naltrexone is prescribed off-label for these uses. Dispensed on prescription following a medical evaluation.

HOW LDN WORKS
Glucose control.
Naltrexone is an opioid receptor antagonist. At 4.5mg it occupies those receptors only briefly, and the leading hypothesis is that the body responds by increasing its own endorphin production. A separate proposed pathway involves reduced inflammatory signaling from glial cells in the central nervous system.
The low dose clears quickly, producing a short window of partial blockade rather than the sustained blockade seen at 50mg.
The working hypothesis is that transient blockade prompts a compensatory increase in endogenous endorphin production after the drug clears.
Research has examined whether LDN reduces microglial inflammatory signaling via toll-like receptor 4. This remains a hypothesis under study.
LDN is typically started low and increased over several weeks. Vivid dreams and sleep disturbance early on are the most commonly reported effects.
